Verruca Worksheet
In today's squeeze on reimbursement and emphasis on documentation, accurate records are imperative. When patients present with multiple lesions, to bill for payment, each lesion must be in the record. If the location and/or size of just one lesion is forgotten between the time of treatment and time of dication or preparation of the record, every 2 or 3 patients treated, this can result in the annual loss of thousands of dollars.
For each patient with more than 1 or 2 lesions, a staff member accompanies the treating physician or PA and the doctor calls out the size and appearance (discrete, diffuse). The assistant can see where the physician is treating, mark the area on the worksheet with a symbol denoting discrete or diffuse, and then record size.
This provides a contemporaneous and accurate record. Each has an assortment of illustrations of common areas treated for each type of lesion.
