3-year full replacement warranty on all parts (excluding lamps) and labor. Lamps carry a 90 day warranty. The cart (HF/Cart) is sold separately.

Shipping Information

Your unit will be shipped via common carrier (BAX Global). The carrier will deliver directly to your home/office and is responsible for removing the unit from the truck and taking it through the front door ONLY.

IMPORTANT INFORMATION ON YOUR HOME PHOTOTHERAPY UNIT

READ THE FOLLOWING IMPORTANT INFORMATION BEFORE ACCEPTING DELIVERY:
The product being delivered to you was thoroughly inspected and carefully packed at the manufacturer. The carrier delivering the product to you assumed responsibility for its safe, complete deliver when they received and accepted the shipment at our warehouse. There are important steps you must take before accepting delivery.

CHECK THE PRODUCT PACKAGING FOR OBVIOUS VISIBLE DAMAGE OR ABUSE
If there is visible damage when the shipment is received (dents, scratches, parts broken loose, tears or holes in the packaging material, etc.) or the sound of broken glass or hardware is heard from inside, note and record the specific damage information on the freight bill/delivery receipt and have the driver sign all copies. Failure to write and adequately describe external evidence of loss or damage may result in the delivery carrier refusing to honor a damage claim later on. CHECK THE GENERAL CONDITION OF THE PRODUCT WHEN UNPACKAGED AS SOON AS POSSIBLE AFTER DELIVERY
Concealed damage may not be evident until the product is unpacked and assembled. When internal concealed damage is found or suspected, immediately contact the carrier's local office and our customer service department at 800-831-6273. Please note: By law, you have only 24 hours from time of receipt to notify a carrier of concealed damage.

CHECK THE NUMBER OF PACKAGES RECEIVED
Check the number of packages indicated on the freight bill/delivery receipt against the actual number of packages being received. If there is a discrepancy between the number indicated and the number of packages received, note and record the shortage or excess on the shipping papers and have the driver sign all copies. Shipping carriers will not honor claims for losses or missing packages if the customer signs the receipt as complete and did not indicate any shortages.

Prescription Information

HF/UVA, HF/UVB, HF/UVBNB is/are only available by prescription. You will need to mail us the original prescription. For faster delivery, you may fax us the prescription (712-323-1156) first and then mail the original. We can ship your order as soon as we receive the fax or original prescription. All prescription orders require an adult to sign for the package. An additional $4.50 fee will be added to your order total for this service.
