Prescription Information
PH-35, PH-36, UV-84, PH-11 is/are only available by prescription. You will need to mail us the original prescription. For faster delivery, you may fax us the prescription (712-323-1156) first and then mail the original. We can ship your order as soon as we receive the fax or original prescription. All prescription orders require an adult to sign for the package. An additional $4.50 fee will be added to your order total for this service.
